LOW COST STUDENT ACCIDENT INSURANCE AVAILABLE
Dear Parents or Guardian,
The State College Area School District does not carry medical insurance on students. The School District does provide you with the opportunity to purchase an Accident Insurance Plan through Monumental Life Insurance Company, an A+ rated company. Due to an injury from an accident, this $500,000 Maximum Benefit coverage will also respond for injuries students sustain and will also respond for school sports (except football) injuries. The coverage includes:
Hospital ServicesHospital Room and Board - Semi-private Usual & Customary Charge Hospital-Misc Services during hospital confinement Usual & Customary Charge Emergency Room Charges Usual & Customary Charge Doctor’s ServicesSurgery Usual & Customary Charge Anesthesia Usual & Customary Charge In Patient Doctor’s Visit Usual & Customary Charge Additional ServicesOutpatient X-Ray, CTI scan, MRI & lab tests other than Physiotherapy Usual & Customary Charge Out Patient Nursing Services Usual & Customary Charge Ambulance Transport – to and from Hospital Reasonable & Necessary Charge Medical Equipment Rental Usual & Customary Charge Out Patient Drugs & Medications Usual & Customary Charge in Hospital or by Written Prescription Dental*(including X-Rays) –due to a covered injury Usual & Customary Charge up to $10,000. Accidental Death $10,000. Dismemberment $Up to $20,000. (based on a schedule)
Conditions First medical expense must be incurred within 90 days of Covered Event Maximum benefit period is five years (unless otherwise noted) Deductible is $0.
Rates School Time Only $ 36. 24 Hour Coverage $131.
