ATTENTION ELEVENTH GRADE PARENTS:

If your student had a physical during their tenth grade year, or over this summer (this includes sports physicals, physicals for driver's licenses, or work permits), or will have a physical during their eleventh grade year, please send a copy to the High School Nurse.  This physical will satisfy the state mandated eleventh grade physical.

ATTENTION INCOMING TWELFTH GRADE PARENTS:

ALL INCOMING SENIORS ARE REQUIRED TO HAVE A DOSE OF THE MENINGOCOCCAL VACCINE (MCV) ON OR AFER THEIR 16^TH BIRTHDAY.  THIS IMMUNIZATION NEEDS TO BE RECIVED BY THE START OF HIS/HER TWELFTH GRADE YEAR.  PLEASE SEND A COPY OF YOUR STUDENT’S VACCINATION RECORD INDICATING THIS VACCINE HAS BEEN RECEIVED BY THE START OF THE 2019-2020 SCHOOL YEAR.

The Pennsylvania School Health Law requires physical examinations of students in KINDERGARTEN, GRADES 6 AND 11, and NEW STUDENTS who have no record of a physician’s exam.  In regard to the 11th grade physical exam, any exam done from the 1st day of 10th grade, through the 11th grade year will meet the requirement.
Physical examinations are offered at school if you are unable to have your child examined by your personal physician. Please complete and return the parent permission form to your student's certified school nurse. Students having a school physical will need to bring a pair of shorts and a short sleeve shirt on the day of the exam.
