K-12 Physical, Dental, Immunization Requirements
The following requirements are mandatory by Pennsylvania School Health Law and must be completed prior to starting school:
Immunization Requirements
Immunization requirements are outlined by the PA Dept. of Health.
Physical Exams
Pennsylvania School Health Law requires physical examinations of:
- Kindergarten students
- New students to the district
- All 6th graders
- All 11th graders
- Any students who have no record of a physician's exam on file
Private physician exam
- We strongly encourage you to take your child to your private physician since he or she can best evaluate your child and make necessary recommendations and provide immunizations as needed. An examination performed 12 months prior to the start of school or anytime during the school year is accepted.
- To meet this requirement, submit one of the following during registration or to the school nurse before the start of the school year:
- Private Physician's Exam Form
- Exam summary from visit or patient portal
Dental Exam Requirements
The Pennsylvania School Health Law requires dental examinations of:
- Kindergarten students
- New students to the district
- All 3rd graders
- All 7th graders
- Any students who have no record of a dental exam on file
Private dental exam
- We strongly encourage you to take your child to your private dentist since they can best evaluate your child and make the necessary recommendations. An examination performed 12 months prior to the start of school and anytime during the school year is accepted.
- To meet this requirement, submit one of the following during registration or to your child's school nurse:
- Private Dental Exam Form
- Copy of visit/billing summary (must include name of provider and date of exam)
Students with no medical or dental insurance
Centre Volunteers in Medicine is an eligibility-based clinic that provides free medical, dental, behavioral health, and case management services to low-income, uninsured residents of Centre County. For more information, call 814-231-4043 or visit their website. Please note that CVIM is not a walk-in clinic, and in order to receive services, individuals must successfully complete an eligibility verification process
