Incident Investigation

The SCASD Safety Committee reviews all accidents and incidents that occur within the district to recommend changes that would prevent their recurrence.  Historically, supervisors have used a paper copy of the incident investigation form which often leads to a delay in information getting to the Safety Committee.  In order to expedite the process, the Safety Committee has digitized the incident investigation form and supervisors are asked to meet with the injured party and fill out the google form providing as much detail as possible within 48 hours of the incident.
