Any student who has a life-threatening food allergy will need to have a new Food Allergy & Anaphylaxis Emergency Care Plan (HS 41) completed and signed by their healthcare provider every school year. The action plan must also be signed by a parent or guardian. Please have all forms completed and submitted at the start of the school year so that we can provide the best care possible to your student.
Any student who has asthma and needs to keep rescue medications at school will need to have a new Asthma Action Plan (HS 24) completed and signed by their healthcare provider every school year. The action plan must also be signed by a parent or guardian. Please have all forms completed and submitted at the start of the school year so that we can provide the best care possible to your student.
Any student who has seizures will need to have a new Seizure Action Plan (HS 55) completed and signed by their healthcare provider every school year. The action plan must also be signed by a parent or guardian. Please have all forms completed and submitted at the start of the school year so that we can provide the best care possible to your student.
Medications
The following over-the-counter medications/treatments may be administered by health room staff with a completed and signed over-the-counter medication form:

Bacitracin antibiotic ointment

Calamine

Hydrocortisone Cream 1%

Metholyptus cough drops

Aloe Vera lotion
The form also allows for you to confirm that your student can apply their own non-aerosol topical sunscreen, as needed.
If your student is prescribed medication that must be administered at any time at school (daily or as needed), the health room must have a signed order from a medical provider (DO, MD, CRNP, PA-C) on file. You must also supply the health room with the medication (we are not able to stock medications in the health room). Please review the SCASD medication policy here.
Physical and Dental ExamsAll kindergarten and new students entering Easterly Parkway Elementary must have an up to date physical and dental examination on record. An examination performed 12 months prior to the start of school and any time during the school year is acceptable.
All third grade students at Easterly Parkway Elementary must have a new, updated dental examination on record. An examination performed 12 months prior to the start of school and any time during the school year is acceptable.
