This form is to be completed to implement the administration of over-the-counter medications in school (grades 6-12) by the school nurse or other licensed health care provider, per standing orders of the school physician.

This form is to be completed to implement the administration of over-the-counter medications in school (grades Kindergarten-5) by the school nurse or other licensed health care provider , per standing orders of the school physician.
